The personal information that we ask for is used to communicate your story to other people on the Internet (World Wide Web), as is normally the case with testimonials of this kind. Your testimonial when accepted by us and made visible on our website, will be viewable to anyone, anywhere who has access to a computer connected to the Internet. By providing us with your testimonial and authorizing us to display your testimonial on www.upcspine.com you acknowledge that you are aware that your testimonial will be made available for display worldwide, can be printed or downloaded and that you have authorized its display and access.
